Registration Checklist

NYC Department of Education

Be sure to bring the following when you register for school.

Required Documents

Proof of your child's age (child's birth certificate, passport, or record of baptism),
Your child's immunization records (if available),
Your child's latest report card/transcript (if available), and
Two (2) of the documents below verifying proof of address:

Acceptable Proof of Address Documents

Lease agreement, deed, mortgage statement for the residence;
A residential utility bill (gas or electric) in the resident's name issued by a utility company (such as National Grid or Con Edison) — must be dated within the past 60 days;
A bill for cable television services provided to the residence; must include the name of the parent and the address of the residence and be dated within the past 60 days;
Documentation or letter on letterhead from a federal, state, or local government agency, including the IRS, the City Housing Authority, the federal Office of Refugee Resettlement, the Human Resources Administration, or the Administration for Children Services (ACS), or an ACS subcontractor, indicating the resident's name and address — must be dated within the past 60 days;
A current property tax bill for the residence;
A water bill for the residence — must be dated within the past 90 days;
Rent receipt which includes the address of residence — must be dated within the past 60 days;
State, city, or other government issued identification (including an IDNYC card), which has not expired and includes the address of residence;
Income tax form for the last calendar year;
Official NYS Driver's License or learner's permit, which has not expired;
Official payroll documentation from an employer issued within the past 60 days, such as a paystub with home address, a form submitted for tax withholding purposes, or payroll receipt (a letter on the employer's letterhead is not adequate) — must include home address and be dated within the past 60 days;
Voter registration documents, which include the name of the parent and the address of residence;
Unexpired membership documents based upon residency (such as neighborhood residents' association), which include the name of the parent and the address of residence;
Evidence of custody of the child, including but not limited to judicial custody orders or guardianship papers; documents must have been issued within the past 60 days and include name of student and address of residence.

Note for Students in Temporary Housing

Students in temporary housing, as defined by McKinney-Vento, are not required to submit documentation (including address, proof of date of birth, and immunization) in order to enroll. Schools must provisionally pre-register the student and then work with the students in temporary housing DOE contact to obtain documentation.

Early Childhood Programs
3-K and Pre-K

Children can start 3-K in the calendar year they turn three.

Children can start pre-K in the calendar year they turn four.

Families can explore options online and join waitlists for programs at MySchools.nyc.

Some 3-K and pre-K programs offer year-round care and education for up to 10 hours a day to children who qualify based on family income and needs—learn more at nyc.gov/PreK or nyc.gov/3K.

Elementary School
Kindergarten to Grade 5

Children start kindergarten the calendar year they turn five.

Most children attend their zoned elementary or middle school—find yours at schoolsearch.schools.nyc.

Have a zoned school and want to attend it? Enroll directly at that school.

Don't have a zoned school or want to explore other options? Contact programs directly or visit a Family Welcome Center (on.nyc.gov/FWC).

Middle School
Grades 6 to 8

Students who have completed grade 5 can start middle school.

Most children attend their zoned elementary or middle school—find yours at schoolsearch.schools.nyc.

Have a zoned school and want to attend it? Enroll directly at that school.

Don't have a zoned school or want to explore other options? Contact programs directly or visit a Family Welcome Center (on.nyc.gov/FWC).

High School
Grades 9 to 12

Students who have completed grade 8 can start high school.

Explore high schools and programs online at MySchools.nyc. Then visit a Family Welcome Center (on.nyc.gov/FWC) to discuss your options and enroll.

For children entering 3-K, pre-K, kindergarten, grade 6, or grade 9:
Visit on.nyc.gov/Enrollment to learn if applications or waitlists are open—during these times, you can use MySchools to apply or add your child to programs' waitlists.
Child Care for Eligible Families

NYC BrightStarts programs provide year-round care for children aged 6 weeks to 2 years old for up to 10 hours a day. Eligibility is based on income and needs. Learn more at on.nyc.gov/BrightStarts.

Find Your District And Zoned School

Most children attend their zoned school for elementary and middle school.

Learn which school district you live in, if you have a zoned school, and that school's contact information by calling 311 or entering your address at schoolsearch.schools.nyc.

If you don't have a zoned school, you can contact schools directly to enroll, or you can visit a Family Welcome Center to learn about your school options.

Find Your Local Family Welcome Center

Family Welcome Centers serve NYC families all year round with enrollment and admissions. We have locations in every borough and are open Monday through Friday all year round. Please check on.nyc.gov/FWC for the most up-to-date information on hours and locations.

When you contact or visit us, please let us know:

  • Your child's first name, last name, and date of birth
  • If support is needed in another language
  • If you (the child's parent/guardian) are deaf or hard of hearing

Use MySchools to Find Schools and Apply

MySchools.nyc is a searchable online directory of NYC public schools and programs, as well as a personalized application system for families.

  • Explore school options any time of year in our public directories.
  • Create an account to apply to school when applications are open.
  • Missed the application period? When waitlists are open, you can use MySchools to add your child to programs' waitlists.
  • Find out when applications and waitlists are open at on.nyc.gov/Enrollment.
  • Learn more about waitlists at schools.nyc.gov/Waitlists.

Who can become a student at MHCS?

  • Receiving English Language services for four years or less at the time of admission to the school
  • Are designated “Multilingual Learners” due to their scores on the NYSITELL and/or NYSESLAT examinations
